I have made rattles similar to this before for friends babies, it adds a personal touch by making the rattle in the shape of the first letter in the new child's name.
-First you sketch the shape of the letter
-cut this out
-pin to your fabric
-cut out the fabric
-hand sew any motif or trim on
-stitch right sides together (leaving a small gap to turn it back out the right way)
-fill with stuffing and insert a couple of bells inside (for the rattle noise)
-then hand sew the gap to finish
